Replacement PADI Card or Certification Fee
For replacement PADI cards be sure to send us a digital photo (head and shoulders no dark glasses) , your name as it appeared on the card, and your date of birth. Also tell us what certification level you are asking to replace. We also use this link for PADI certification fee.
